1.) Every person under the age of 18 needs to be accompanied by an adult in the exam rooms at every visit including contact lens checks. 2.) Payment is due at the time of service unless prior arrangements have been made. 3.) As a curtesy to our patients we do file vision and medical insurance. 4.) We do make a copy of your drivers license and vision and medical insurance cards on your first visit. The reason we make a copy of your medical insurance is because if you have a medical condition (ie: eye infection) we need to file that under medical your vision insurance will not cover that visit. The reason we make a copy of your drivers license is because we need a photo ID to prove identity and to file insurance. 5.)
